Declaration of Domicile 

This document proves residency in Broward County, Florida. It is often used to enroll children or college students in school, to establish eligibility for Homestead Exemption, or as the first step toward establishing legal residence in this State. The customer must present proof of identity with a picture, such as Florida Driver's License or passport. People who do not drive can use state-issued identification cards. Resident aliens must have permanent green cards. The form can be picked up in this office, or downloaded from this site, and completed and notarized elsewhere, then mailed or brought in to the office, or completed and notarized in the office. It costs $10.00 to record the document, if it is notarized before it is brought in. If it must be notarized in the office, there is an extra $10.00 charge for each name notarized. Husband and wife or Registered Domestic Partners can use one form to apply. All other individuals must submit separate forms. Please do not sign the Declaration of Domicile until you are actually in front of the Notary Public.
